Protected By Illness. The Plague Spreader can only suffer from one disease at a time, making them Immune to other diseases while they’re afflicted by their current disease. In addition, rolls against the Sickened condition are made with a +5 bonus.

Disliked. Once the Plague Spreader has been identified as such, they get a -2 penalty to their CHA checks.

Diseased Corpse. Any time a creature comes within 30 ft of the Plague Spreader’s corpse, if its corpse is not sealed in a coffin or otherwise contained, then the Plague Spreader’s corpse makes a +4 attack against the target creature’s FORT. On a hit, the creature contracts the disease that the Plague Spreader had.

Actions
On each of the Plague Spreader’s turns, it may take one of the following sets of actions.
  • Move and take one Tier 1 action.
Tier 1 Actions
Move. The Plague Spreader can move up to their speed.

Dagger. Thrown 20 ft One-Handed Weapon. +0 vs AR. Hit: 2 (1d4) sharp.

Cough. Reach 5 ft Natural Weapon. +2 vs FORT. Hit: 6 (1d8+2) necrotic and the creature becomes Sickened until the end of their next turn. If they become Sickened in this way twice, they gain the same disease that the Plague Spreader has.
